Confirmation of Service Hours Request

Do you need confirmation that you volunteered at Habitat for Humanity or completed community service? If so, please fill out the form below so that we can prepare a personalized letter for you verifying your service. If you are completing community service, please read through our Community Service Procedures for additional information you may need.

Letters of confirmation will be sealed in an envelope and left for pick up at the cash register of the Habitat for Humanity ReStore. If you are completing community service, a copy of your Community Service Log Sheet will also be included, listing the dates and times that you volunteered. You may also request that your letter be faxed or emailed somewhere for convenience.

ONLY submit this form when you are close to needing a confirmation letter or completing your hours, not as soon as you begin volunteering. If you have additional questions regarding how to get confirmation of service hours, please email volunteer@habitatspringfieldmo.org.
